Medical Certificate from Public Service
INTRODUCTION
The following content is an example of applying a medical certificate from Prince of Wales Hospital. For details, please refer to the page "Medical Report / Patient’s Information" on the website of PWH. If you wish to apply that from other public hospitals, please refer to "Guide to Application for Medical Reports" of the Hospital Authority, or website of the respective hospital.
Doctors at public hospitals generally do not write any medical certificates during a consultation. A formal application is required.

According to PWH's Notes of Application for Medical Report for Medical Report / Patient's Information, a medical report takes approximately 8 weeks to complete.
IF APPLYING WITH CRITERION 2 (SURGERY + HORMONES + LIVING + GENDER DYSPHORIA + DECLARATION)
🔔 Attention:
ImmD requires the issue date of medical certificate for hormones + living, to be the same date OR later than the submission date of Form ROP73.
Please read the details and recommendations below.
Submit Form ROP73, on the same day
a doctor issues a medical certificate for hormones + living.
Otherwise, first submit Form ROP73. They then ask a doctor
to issue the medical certificate for hormones + living.
If ImmD's Form ROP73 is submitted after receiving the medical certificate mailed by PWH, ImmD would

Request for an additional medical certificate for hormones + living: Same content with a later issue date

To certify one is consistently using hormones, and living in one's gender identity, on and before the submission date of Form ROP73.

If your surgery is done by PWH, and have been on hormones + living for 2 years or more by that time,

Immediately after the surgery, please ask PWH doctor to complete every part of a medical certificate, including surgery + hormones + living + gender dysphoria; OR
If your surgery is not done by PWH, and have been on hormones + living for 2 years or more by that time,

Immediately after the surgery, please ask the doctor to complete every part of a medical certificate, including surgery + hormones + living + gender dysphoria; OR
When submitting Form ROP73 to ImmD, attach a copy of HKID, and do not attach medical certificate that needs to be written by PWH.

After that, submit "Medical Report / Patient's Information Application Form" to PWH.

Generally, 2-4 weeks after submission of ImmD's ROP73, ImmD would mail a letter to one's address written on Form ROP73. The letter shows an application number, and remaining document(s) to be submitted. Please follow instructions on the letter to email, mail, or fax a copy of the document(s). ImmD usually specifies the remaining documents to be submitted within 2 weeks. One may inform ImmD that the "Medical certificate from the Hospital Authority takes approximately 8 weeks".

After receiving the medical certificate mailed by PWH, please submit it to ImmD as soon as you can. This usually would not affect the application); OR
If needed, one may ask any doctor registered in Hong Kong to complete Annex 2 of ImmD's Guideline.

EXAMPLE OF COMPLETION OF PWH'S MEDICAL REPORT / PATIENT'S INFORMATION APPLICATION FORM
After approval of the Disability Allowance, submit the application form for the Personalised Octopus with "Persons with Disabilities Status".
