FORMS AND/OR SAMPLE FOR REFERENCE OF MEDICAL CERTIFICATE IN IMMD'S GUIDELINE

If your doctor practises in Hong Kong

  • Please ask your doctor to fill in Annex 2


If your doctor practises outside Hong Kong

  • Please ask your doctor to fill in Annex 3

Please print a copy of the English version of Annex 2 or Annex 3 of ImmD's Guideline, for your doctor to fill in information.

  • If your doctor would like to write a separate letter, please ask them to use exact wordings on the English version of Annex 2 or Annex 3 of ImmD's Guideline.

    • E.g. write "removal of the penis and testes" and not "orchidectomy and penectomy", "construction of a vagina" and not "vaginoplasty".

DATES OF SURGERY(IES), AND START OF HORMONES + LIVING

Dates of surgery(ies), and (If applying with Criterion 2) start of hormones + living

  • Recommended for your doctor to write a date with year, month, and day.

MEDICAL CERTIFICATE FOR GENDER-AFFIRMING SURGERY(IES)

Right after your surgery, please ask the surgeon to write this medical certificate, and collect relevant documents (if any).

 

If needed (E.g. unable to obtain a medical certificate from your surgeon, your original medical certificate is rejected by ImmD due to incomplete information), you may ask any doctor registered in Hong Kong to complete Annex 2 of ImmD's Guideline.

  • Please produce relevant records (if any). The doctor may also do a simple examination for you.​

  • The doctor is not required to fill in the items for hormones and living in Annex 2 / 3.

     

    The doctor only needs to fill in the relevant part(s) for the surgery, i.e.

    • Part A - Item d;

    • If the surgery was not performed by this doctor writing the medical certificate, the doctor also needs to fill in Part B.

MEDICAL CERTIFICATE FOR HORMONES + LIVING + GENDER DYSPHORIA (ONLY REQUIRED IF APPLYTING WITH CRITERION 2 OR 3)

Please contact the doctor who follows up for you to write this document.

If needed, you may ask any doctor registered in Hong Kong to complete this medical certificate.

  • Please produce relevant records, e.g. prescription records, blood reports.​

🔔 Attention:
ImmD requires the issue date of medical certificate for hormones + living, to be the same date OR later than the submission date of Form ROP73.

  • Medical certificate from private service / doctors outside Hong Kong

    • Recommendation: Submit Form ROP 73 with the medical certificate for hormones + living to ImmD, on the same day a doctor issues the medical certificate for hormones + living.


(Otherwise, first submit Form ROP 73. They then ask a doctor to issue the medical certificate for hormones + living. They would be able to submit the remaining documents after receiving a letter from ImmD)

  • 2-4 weeks after the submission of Form ROP73, ImmD would mail a letter to one's address written on Form ROP73.

     

    The letter shows an application number, remaining document(s) to be submitted (if any), and

    • Requests for an additional medical certificate for hormones + living: Same content with a later issue date

      • To certify one is consistently using hormones, and living in one's gender identity, on and before the submission date of Form ROP73.

     

    Please follow the instructions on the letter to email, mail, or fax a copy of the document(s).

    Submit in 1 medical certificate in total.

    • Containing details on surgery, and hormones + living + gender dysphoria, issued by the same doctor. OR

     

    Submit in 2 medical certificates in total

    • One for surgery.

      • Written by the doctor that performed the surgery.

    • One for hormones + living + gender dysphoria.

      • Written by another doctor.

ANNEX 3: MEDICAL CERTIFICATE ISSUED BY A DOCTOR OUTSIDE HONG KONG

Please include the following information for "Medical qualication" on Annex 3.

  • Registration number of the doctor

  • Medical qualification, its recognizing organization and country/region

(Please note that a hospital/clinic where the doctor works at, although may be included, but this is not the same as the doctor's medical qualification)

ImmD only accepts medical certificate issued by a doctor. In case you are followed by by a Doctor of Nursing Practice, or a Doctor of Osteopathic Medicine, please still ask a doctor to write your medical certificate.
